Studying for the NCLEX means covering a lot of ground — safe, effective care; pharmacology; prioritization; and the drug math that trips up even strong students. This page is a starting point: a handful of practice questions to gauge where you stand, a look at what changed (and didn't) for 2026, and links to the two free tools on this site.

What's on the NCLEX in 2026

The NCLEX test plan updated on April 1, 2026, but the shape of the exam didn't change much. It's still computer adaptive — the exam adjusts question difficulty based on how you're answering, and it ends once the algorithm is confident you're above or below the passing standard, not after a fixed number of questions. The Next Generation NCLEX (NGN) item types introduced in 2023 are unchanged: case studies, bowtie items, matrix items, drag-and-drop, and highlight questions that ask you to apply clinical judgment across several linked questions rather than answer one fact in isolation.

Content weighting is also unchanged. Pharmacological and Parenteral Therapies remains one of the largest categories, alongside Management of Care and Physiological Adaptation. The main 2026 update was language, not substance: some categories were renamed for clarity, "substance abuse" became "substance misuse," and guidance on equitable, person-centered care was added throughout.

For drug math specifically, the highest-yield areas are still the same: basic dosage, IV flow and drip rates, weight-based pediatric dosing, and — heavily tested — safe dose range checking and rounding conventions (a nurse who calculates correctly but doesn't catch an unsafe order hasn't actually done the job). A) A patient 2 days post-op with a temperature of 100.4°F
B) A patient with a new onset of slurred speech and facial drooping

B. Slurred speech and facial drooping are classic stroke warning signs (part of the FAST assessment) and require immediate evaluation — every minute of delay matters for a possible ischemic event. The other three are important but not time-critical in the same way.

3. A patient starting lisinopril should be monitored for which of the following?
A) Hypoglycemia
B) Hyperkalemia
C) Hyponatremia
D) Hypocalcemia

B) Hyperkalemia. ACE inhibitors like lisinopril reduce aldosterone release, which decreases potassium excretion by the kidneys — a well-known and frequently tested side effect.

4. A child weighs 15 kg. The order is amoxicillin 40 mg/kg/day divided every 8 hours. How many mg should the nurse give per dose?

200 mg per dose. 40 mg/kg/day × 15 kg = 600 mg/day. Every 8 hours means 3 doses/day, so 600 ÷ 3 = 200 mg/dose. Weight-based pediatric dosing is one of the most common calculation types on the exam.

5. On a Next Generation NCLEX case study, a nurse is asked to select actions to take, actions to monitor, and a potential condition — all about the same patient scenario. What is this item type testing?

Clinical judgment across a sequence of decisions, not an isolated fact. NGN case studies are scored on whether your choices form a coherent, safe plan of care as the scenario unfolds — later questions can depend on how the patient responds to earlier ones. The best preparation is practicing full scenarios, not just individual facts in isolation.

A note on how to actually use practice questions

Getting a question right for the wrong reason is worse than getting it wrong for the right reason — the first teaches you nothing, the second teaches you exactly what to fix. Read the rationale even (especially) when you got it right, and if a topic keeps tripping you up, that's the signal to go back to the formula or concept, not just drill more questions on it.

Yes. Do I need to know both proportion and dimensional analysis methods?

You only need one method that you can execute reliably under pressure — most nursing programs teach dimensional analysis because it scales cleanly to multi-step conversions, but proportion works fine too. What matters more than which method you pick is practicing it enough that you're not reconstructing the logic from scratch on every question.

Are NAPLEX and NCLEX drug calculations the same?

The core arithmetic overlaps heavily — ratios, unit conversion, dose calculation — but NAPLEX (the pharmacist licensing exam) goes considerably further into pharmacy-specific calculations like compounding, percentage and ratio strength, pharmacokinetics, and dose-equivalence conversions between drugs. A nursing candidate doesn't need that depth, but the shared fundamentals mean NAPLEX practice material is still useful supplementary practice for NCLEX prep, and vice versa.
